I feel your pain after wasting 2 weeks, 12 email, 3 faxes and 2 photo ids on those lousy bastards. Just call and they'll pretty much do anything you ask if you have the patience to wait in the queue with the most insipid hold music I've heard in a while. I miss the good old days of just being able to mail the form in and be done with it. *grumble*
