Swen is what finally got me to set up qpsmtpd (which does need a catchier name if it's ever going to make the big time) this week. Of course things like this make me want a "line received" hook to avoid having to get the whole message, but I suppose that wouldn't be very performance-friendly.

  No, but we're thinking of adding something along the lines of headers received or perhaps even MIME part received.
  
  And besides, it's not as though qpsmtpd is terribly optimal in the first place - though I think we'd try and make it as optimal as possible.
