I find this site [perl.org] quite good.

I have set up a small slash installation. I started changing the templates to get rid of the tables and introduce some CSS goodness and then discovered how big a job that was. I'm kind of stalled on that project.

Depends on what you think is “good”, but this one [morrissey-solo.com] has been Slash-based for a long time and is very active indeed.

slashcode project seems to be pretty active. the latest slashcode was released 2005-03-29 http://sourceforge.net/project/showfiles.php?group_id=4421&release_id=97896 [sourceforge.net]

I've had Taperfriendlymusic.org [taperfriendlymusic.org] running slashcode for over a year and a half now. I haven't seen slashdot level traffic, but I hacked the auth system so that users who sign up automatically get an ftp account on the site.
