the one time I've seen OS X seriously fail on my computer is when the system runs out of space for paging. usually accompanied by an alert dialog of the sort "please quit something quick". (I was testing a nasty bug in mozilla, when this happened). the characteristic was a gradual slowing of everything, apps starting to lock up, eventually unable to to even move windows around in aqua apps. grinding hard drive.
